As per report downloaded from Dengue Dashboard Disease Tracking and Response System, you didn’t submit even a single anti-dengue activity on 25.01.2022. You were issued Explanation Letter vide No. 5266 dated 26.01.2022 & vide no. 5287 dated 27.01.2022 and personal hearing was fixed on 28-01-2022 at 03:00 PM. But you did not appear before the undersigned on the fixed date and time & also did not submit any written reply.
You are exclusively held responsible for above-mentioned charge. THEREFORE, you are offered an anon again opportunity of personal hearing and hereby called upon in person to explain your position in office of the undersigned on 29.01.2022 at 03:00 PM, failing which it shall be presumed that you have nothing to say in your defense and the recommendations as per PEEDA Act 106 will be submitted to the competent authority for further action.
