Immediate Reporting of Unforeseen Incidents in Schools

District Education Authority Rahim Yar Khan has issued instructions for the immediate reporting of unforeseen incidents, accidents, emergencies and other unusual events in public schools and offices. According to the notification dated 1st October 2026, all concerned District Education Officers, Deputy District Education Officers and Heads of Institutions are required to ensure that incidents such as fire, building collapse, security-related occurrences, serious injuries, loss of life and natural calamities are reported to the competent authority without any delay.

I am directed to refer to the subject cited above and to convey that the competent authority has directed that all unforeseen incidents, accidents, emergencies, natural calamities, fire, building collapse, security-related occurrences, serious injuries, loss of life, or any other unusual event occurring in any public school or office under the administrative control of the Education Department shall be reported immediately and without any delay to the competent authority.
- All District Education Officers and Deputy District Education Officers are directed to ensure strict compliance with these instructions. They shall further ensure that all concerned officers and Heads of Institutions are properly sensitized and instructed.

In case of occurrence of any unforeseen incident/accident or unusual event, initial information shall immediately be communicated through the quickest available means without waiting for the formal written report. A detailed written report shall contain:
- Name and EMIS Code of the school/institution.
- Date, time and place of the incident.
- Brief description of the occurrence.
- Details of injuries, casualties or damage.
- Immediate action taken by school and district administration.
- Current status and further assistance required.
- Concerned District Education Officers and Deputy District Education Officers shall personally ensure the accuracy and timely transmission of information. No incident shall be concealed, suppressed or delayed. Any delay, concealment or failure to report shall be viewed seriously.
- These instructions may also be circulated to all concerned Deputy DEOs, AEOs and Heads of Institutions for immediate compliance.
- You are therefore directed to ensure strict compliance of the above instructions and ensure that no lapse occurs in immediate reporting of any unforeseen incident/accident or unusual event.
