Attested computerized Nikah Nama and attested death certificate of the husband issued by the concerned Union Council. |

Divorced or Khula

Attested computerized Nikah Nama and attested divorce/separation certificate issued by the concerned Union Council. Legal termination of marriage and a computerized separation certificate are required. | |

Differently abled
|
Attested disability certificate issued by the competent Assessment Board for the affected teacher, spouse or child, as applicable. FRC/Form-B is required for disability of family member. |

Medical grounds

Medical certificate from the Medical Board of the concerned DHQ Hospital, with findings on the prescribed Disease Assessment Proforma. The teacher undergoes examination through the prescribed referral process. |

Wedlock
(Spouse must be Govt. Employee)

Attested computerized Nikah Nama and attested copies of the spouse’s CNIC, domicile and current posting order / letter from the authority. |

Circumstantial single parent
(Spouse must be working overseas)

Copies of the spouse’s visa and passport, and the applicant teacher’s affidavit. The spouse must be abroad to earn a livelihood and the teacher must be the sole caregiver of one or more children. |

Attested copy of Nikah Nama, husband’s CNIC and residential address supported by ownership documents or utility bills. Applies to marriage-related shifting of residence outside Tehsil/District; available once in the whole service. |

- For Wedlock, spouse must be Government Employee.
