Affidavit Provide the statement on stamp paper with two witness. that all of your documents are not fake, if found, you ‘ll be held responsible.

Medical Report: Original Medical Report may be provided from MS DHQ Hospital.

Charge Report: Charge Report may be provided from concern school duly signed by Head of School.

Order Copy: Copy of order may be provided.

Challan copies: Submit the fee for verification of Certificates / Degrees in bank and provide clear copy of Challan.

Duty certificate: Duty Certificate may provide duly signed by Head of School.

CNIC Copy: Attested copy of CNIC.
