I am directed to refer to the subject captioned above and to inform you that the Chief Secretary, Punjab being the Competent Authority has directed that the data regarding vaccination of Government Employees be collected by the Welfare wing, S&GAD.

In pursuance of the above said, I am further directed to request you that the Covid-19 vaccination details of the employees working in your respective department may be shared on the format given below and soft copy may be sent on the email address sowelfare1@gmail.corn, within 02 days, positively:
