- NADRA family registration certificate
- Undertaking for recovery of any payment.
- Option from for direct pension through bank dully verified be the concerned bank.
- Specimen signature/Thumb and finger impression of applicant attested by concerned officer.
- Affidavit for recovery of any over payment
- Copy of pension book
- Non re-marriage certificate dully verified by the DDO concerned
- Lawful widow/widower certificate
- Descriptive roll
- Succession certificate
